Term Life Insurance

Eligible employees:  full-time or part-time (at least 30 hrs/wk)
Effective date: first day of the month coinciding with or following enrollment 
Employee cost:  $0 

Life insurance benefit:  the amount of the employee’s annual, regular pay, rounded up to the nearest $1,000 and not to exceed $150,000.  

Accelerated death benefit:  an employee who is terminally ill and meets eligibility requirements while insured by the plan may request advance payment of up to 50% of the life insurance benefit.   

Benefit amounts are reduced upon reaching age 65.  The life insurance policy may be portable upon an employee’s separation from employment, at the rate determined by the carrier at that time.

Accidental Death & Dismemberment ("AD&D") Insurance

Eligible employees:  full-time or part-time (at least 30 hrs/wk) 
Effective date: first day of the month coinciding with or following enrollment 
Employee cost:  $0 

Accidental death benefit:  the amount of the employee’s annual, regular pay, rounded up to the nearest $1,000.  Should an employee die while in a private passenger vehicle, an additional 10% seatbelt benefit (capped at $25,000) and/or a 5% airbag benefit (capped at $5,000) may be paid.   

Education benefit:  up to $24,000 if applicable.

Accidental dismemberment benefit:  varies based on circumstances.    

Benefit amounts are reduced upon reaching age 65.  The AD&D policy may be portable upon an employee’s separation from employment at the rate determined by the carrier at that time.

Voluntary Life and/or AD&D Insurance 

Eligible employees:  full-time or part-time (at least 30 hrs/wk) 
Effective date: first day of the month coinciding with or following enrollment 
Enrollment:  within 30 days of start date or during annual open enrollment 
Employee cost:  full cost of selected coverage  

In addition to the University-sponsored life and AD&D policy, an employee may elect to purchase additional life and/or AD&D coverage.

Long-Term Disability Insurance

Eligible employees:  full-time or part-time (at least 30 hrs/wk)
Effective date: first day of the month coinciding with or following enrollment 
Employee cost:  $0  

Long-term disability benefits:  monthly income totaling 60% of the employee’s regular monthly pay.  The maximum monthly payment is calculated by combining any income benefit received from Social Security, Worker’s Compensation, or any other group benefit plan and may not exceed $6,000 per month.  

Additional benefits:  rehabilitation and return to work assistance, dependent care expense benefit, and lump sum benefit equal to 3 months of gross disability payment upon meeting eligibility requirements. 

Retirement contribution benefit:  if eligible, the benefit will equal the University’s TIAA contribution rate, plus the employee’s TIAA contribution rate base on the amount of the employee’s last regular pay.  If the employee had been making any voluntary contributions to TIAA, those contributions will cease.   

The elimination period is 180 days, and benefits are reduced at age 62.

Travel Accident Insurance

Eligible employees:  full-time or part-time (at least 30 hrs/wk) between the ages of 18-70
Effective date:  first day of the month after the employee’s first full month of continuous employment 
Employee cost:  $0  

In the event of death or accidental dismemberment while traveling off campus for University-related business, a benefit of up to $100,000 will be paid.  

International Travel Insurance

Eligible employees:  full-time or part-time (at least 30 hrs/wk)
Effective date:  upon hire 
Employee cost:  $0  

Employees who travel internationally and within the coverage territory for University-related business will have coverage for medical, personal, travel, and security assistance, including evacuation and repatriation of remains.  Please visit the Human Resources Office before traveling internationally.
