Online Reporting Forms
Have you witnessed or experienced something that you want to report?
The following online reporting forms, sometimes referred to as complaints, provide one way for individuals to report non-emergency behavior to the University. Following your submission of the report, a campus administrator will receive information about your concern and take appropriate action.
To report an emergency, you should notify University Police immediately by calling 740-826-8155.
Reports are reviewed by University administrators during normal, business hours. You are encouraged to notify a Resident Assistant or University Police if you wish to report an urgent matter after hours.
If you choose to submit an anonymous report (meaning that you do not disclose your identity), please be aware that the University's response may be limited, depending on the circumstances.
