Online Reporting Forms

If you have experienced or witnessed misconduct or are concerned about another person, this page will help you file a report, complaint, or grievance with the appropriate University administrator.

To report an emergency, you should notify University Police immediately by calling 740-826-8155.

The following online forms provide one method for individuals to report non-emergency behavior to the University.  People may also refer to these reports as complaints or grievances. 

Within a reasonable time following submission of the report, a campus administrator will review the information that you have provided and take appropriate action.  Because reports are reviewed during regular, business hours, you are encouraged to notify University Police or a Resident Assistant if you wish to report an urgent matter after hours.  

If you choose to submit an anonymous report (meaning that you do not disclose your identity), please be aware that the University's response may be limited, depending on the circumstances.
