Requesting Accommodations

Students are required to request accommodations annually for supports such as academic (for in-person or online courses), housing, and ESA accommodations. Temporary accommodations are approved on an as-needed basis.

Muskingum University is committed to providing equal access to individuals with qualifying disabilities. Student Accessibility Services provides reasonable accommodations and academic support to students with long-term or temporary disabilities. Each student’s requests will be evaluated on an individual basis based on documentation and functional limitations.

There are a variety of reasonable accommodations available to qualifying students. The process is different for the following types of accommodations; please review each closely to ensure that the proper steps are completed in the request process. Approved accommodations are valid for the academic year in which approval was requested. Prior to the start of the next academic year, students are required to reapply for accommodations. 

Academic Accommodations (including testing accommodations)

1. Complete the Accommodation Request Form.

2. Submit documentation(s).

  • Documentations should be recent being within the last 2-3 years
  • Documentations should be provided or completed by a licensed individual (ex. doctor, therapist, etc.) or from a previous school of enrollment
  • Documentations should contain the following three details:
    • The disability should be identified/explained
    • The impact of the disability on the academic learning environment
    • The recommendations for accommodations needed to be successful in the academic learning environment
  • If needed, the Disability Verification Form can be used to complete this step.

3. Schedule and attend a meeting with a Student Accessibility Services staff member.

  • Meetings can be scheduled through email, phone, or in-person on the ground floor of Walter Hall

Housing Accommodations

1. Complete the Housing Accommodation Request Form.

2. Submit documentation(s).

  • Documentations should be recent being within the last 2-3 years
  • Documentations should be provided or completed by a licensed individual (ex. doctor, therapist, etc.)
  • Documentations should contain the following three details:
    • The disability should be identified/explained
    • The impact of the disability on the ability to house without accommodations
    • The recommendations for accommodations needed to be successful
  • If needed, the Disability Verification Form can be used to complete this step.

3. Schedule and attend a meeting with a Student Accessibility Services staff member.

  • Meetings can be scheduled through email, phone, or in-person on the ground floor of Walter Hall

*Housing accommodations are limited and based on a ranking of student needs. Therefore, continuing students are required to submit their requests for housing accommodations by the March 1 deadline to be considered for accommodations in the following fall and spring semesters. New students have a July 1 deadline for freshmen housing accommodations. Any requests beyond the established deadlines will be waitlisted and considered for temporary housing accommodation approval pending availability. Students are not guaranteed housing accommodation approval despite completing the three required steps*

Emotional Support Animal Accommodations

1. Complete the ESA Request to Register Form. The ESA Roommate Agreement Form can be uploaded to the ESA Request to Register or be submitted separately. 

2. Submit documentation(s).

  • Have a licensed individual (ex. doctor, therapist, etc.) complete the ESA - Request for Information Form
  • Provide a current and up-to-date vaccination record for the requested ESA
  • Provide pictures of the ESA

    • Right side image

3. Schedule and attend a meeting with a Student Accessibility Services staff member.

  • Meetings can be scheduled through email, phone, or in-person on the ground floor of Walter Hall

*Emotional Support Animals can be requested at any point, but may impact roommates. Therefore, students are strongly encouraged to complete the request process prior to the start of the academic school year.*

Temporary Accommodations

1. Complete the Temporary Accommodation Request form and submit documentation(s).

  • For athletic concussions, notification from the athletic department qualifies as meeting the documentation requirements
  • For all other injuries, documentation from the treating medical provider is needed

2. Schedule and attend a meeting with a Student Accessibility Services staff member.

  • Meetings can be scheduled through email, phone, or in-person on the ground floor of Walter Hall

Timeline

From the time that the accommodation request process (completing the requested documents, providing documentation, and meeting) is completed, Student Accessibility Services may take 7-10 days to review the documents and provide final notification of the results of the review. Additional information may be requested throughout the review process. Therefore, students should monitor their Muskingum University email for communications related to the request for accommodations. 

Process Completion

Once all steps are completed in the request process, a formal accommodation notification letter will be emailed to the student and appropriate parties (instructors, residence life, etc.). Accommodations are effective as of the date that the accommodation notification is emailed. Accommodations are NOT retroactive, meaning that accommodations will not be considered for coursework, exams, absences, etc. prior to the established date of the accommodations. Therefore, it is recommended that students complete the accommodation request process prior to the start of the new academic year to ensure that all accommodations are in place at the beginning of the semester.
