Technology Asset Relocation-Reassignment Request Form

If this is a Reassignment (person leaving department or the employment of the University), then CNS needs to know what to do with their data (emails and/or voicemails).


Computer setup is dependent upon the assignment of the new employee's electronic ID (email user ID). This can not be done until HR has sufficient paperwork to add the employee into their system. Thus, the sooner HR is able to add them into the system, the sooner we can have their computer on their desk.
