Important Forms

For your own convenience a variety of forms are listed below along with the uses for each form. The forms can be downloaded for use.

[Student Athlete Handbook]
This is the Handbook published by the Muskingum University Athletic Department.

[Information for Medical Providers]
This document is useful for physician offices that care for our athletes. The information includes helpful contacts and the necessary insurance coverage provided by Muskingum University.

[Assumption of Risk]
This form is completed annually by student athletes. The document informs athletes of safety provisions made and the recognition on the part of the student athlete of the risk of injury that comes with athletic participation.

[Notice of Privacy Protection]
This document is completed annually by student athletes. The form outlines the privacy policy with regard to the Athletic Training Department

[Medical Care for Minor-Aged Athletes]
This form is signed by the parents of athletes who have not yet reached their 18th birthday. The completed document gives permission to the Athletic Training staff to perform limited injury and illness evaluation and care.

[Referral Form]
This form is used by the Athletic training staff to communicate with referrals to medical professionals that occurs off-campus.

[Wellness Center Medical History]
This form provides medical history information including immunization records to our University Wellness Center. The form is typically sent to incoming students the summer prior to arriving on campus. The form can be FAXed directly to the Wellness Center at 740-826-8151.

[Banned Substance Exemption Form]
This form is used by physicians when assisting the Athletic Training staff in obtaining a NCAA waiver for the athlete use of a banned substance. The form is returned to the Athletic Training staff and kept on file.

[Authorization to Release Medical Records]
